Ethnic rhinoplasty focuses on surgically altering the nose without diminishing a patient’s ethnic characteristics. People of African American, Hispanic, and Asian descent have nasal features unique to their ethnicities. Mediterranean, Middle Eastern and Jewish people have different nasal characteristics and cosmetic concerns. The goal of ethnic rhinoplasty, as with all nasal surgery, is to create a symmetrical, aesthetically pleasing nose that suits each patient’s face.
Ethnic rhinoplasty takes into account the specific internal and external features of each patient’s nose. Ethnic noses may require significant alteration of the internal structures in order to change the external appearance. African American, Hispanic, and Asian patients often have rounder, less protrusive nasal tips. The base of the nose and nostrils may appear wide.
Specialized techniques include plastic surgery to change the shape of bone and cartilage inside the nose, grafting cartilage from elsewhere on the body to extend the nasal tip, and narrowing of the nostrils. Significant internal changes like these will have an impact on the function of the nose. Therefore, great care needs to be taken to maintain optimal breathing function and open airways. In addition to reshaping the nose, rhinoplasty can correct any obstructions or abnormalities of the nose.
Specialization and experience with ethnic rhinoplasty is essential to achieve results that suit each patient’s face. After all, every person’s nose is different. A cookie-cutter approach will not yield pleasing results for every patient. Individualized assessment and a surgery plan tailored to each patient are the basis of any successful surgery.
The nasal proportion guidelines that plastic surgeons in North American are taught have not, traditionally, taken different ethnicities into account. Therefore surgeons like Dr. Moradzadeh have undertaken to learn techniques to recognize and accommodate ethnic features to achieve a result that retain the patient’s ethnicity. His goal is to embrace the cultural identity of the patient while remembering that beauty is cross-cultural. Creating a subtle balance, and maintaining some ethnic characteristics provides the most desirable result.
A functional rhinoplasty may be performed under different types of anesthesia, depending on patient and physician preference, including local, intravenous sedation (i.e. “twilight”) or general anesthesia. Incisions are made to allow access to the underlying cartilage and bone. When these incisions are hidden completely inside the nostrils, the procedure is referred to as “closed”. Often, a small incision may need to be performed at the base of the nose to improve exposure, in which case it would be an “open” functional rhinoplasty.
Depending on the patient’s need, cartilage and bone may need to be grafted to help gain support. These cartilage grafts are often taken from the nasal septum itself during the septoplasty portion of the procedure, although sometimes ear or even rib cartilage may need to be harvested. Further reshaping or grafting may need to be performed with sutures to the cartilage. “Osteotomies” – or deliberate fracturing of the nasal bones – may often be performed to straighten the nasal bones themselves.At the conclusion of the procedure, the outside of the nose is often taped and some form of external cast may be applied. Dr. Moradzadeh rarely uses packing allowing for maximal comfort.
In upper eyelid surgery, the surgeon first marks your natural eyelid crease. This is done to keep the scars invisible along these natural folds. The incision is made, and excess fat, muscle, and loose skin are removed. The amount removed is tailored specifically to each individuals needs. In some cases, fat may be added to restore a more youthful appearance to areas of hollowing. The surgeon uses fine sutures to close the incisions — and minimize the visibility of any scar.
In lower eyelid surgery, the surgeon makes the incision in an inconspicuous area along the lashline of the lower lid. Excess fat, muscle, and skin are then trimmed away before the incision is closed with fine sutures. Eyelid puffiness caused primarily by excess fat may be corrected by a transconjunctival blepharoplasty. In this case, the incision is made inside the lower eyelid, and excess fatty material is conservatively removed.
When sutures are used to close this kind of incision, they are invisible to the eye. They are also self-dissolving and leave no visible scar. In some cases, the fat will be re-arranged (fat transposition), this allows for moving the puffy fat into an area that has a hollow or dark appearance to create a more well-rested, natural appearance. Under normal conditions, blepharoplasty can take from one to three hours.
Both the area from which the fat is taken and the treatment site are anesthetized with a local anesthetic. Using a small needle attached to a syringe, the surgeon removes the fat from a donor site — an area where the patient’s fat is most tightly packed, such as the abdomen or thighs. Once removed, the fat is processed to remove excess fluids and then re-injected using blunt-tipped micro-cannulas applied to various regions and depths of the face. This process may be repeated until the desired correction has been achieved.
Harvesting the fat cells for reinjection is often performed in conjunction with other procedures, such as liposuction or facelift. When fat is harvested during another procedure, it is often possible for the surgeon to harvest a larger amount of fat, and then to freeze and store the fat for future injections should they become necessary.

Through cosmetic rhinoplasty, the nose can be made larger or smaller; the angle of the nose can be changed in relation to the upper lip; the tip of the nose can be altered; or other imperfections like bumps, indentations and other defects can be corrected.
For many years, rhinoplasty was the only available option for patients looking to improve the size and shape of their nose. But recently non-surgical methods have been developed that allows patients to avoid the common risks associated with nose surgery, such as scarring, difficulty breathing through the nose, poor cosmetic results, and long recovery times and discomfort.
Non-surgical rhinoplasty can take less than an hour to perform and does not require anesthesia. Dermal fillers such as Juvederm, Restalyne® or Radiesse® are used to even out the shape of the nose. Non-surgical rhinoplasty is non-invasive, so there are no stitches, casts or scar tissue to be concerned with. Patients can return to their normal routine immediately after the procedure. There may be some swelling and bruising, but it is usually minimal. The results of non-surgical rhinoplasty can last up to 12 months and may need to be repeated.
